Definition
Documented information is information required to be controlled and maintained by an organization and the medium on which it is contained. It includes information that is maintained (documents) and retained (records).
Modern ISO management system standards use this term instead of older separate "documents and records" wording in many places.
Control expectations
Organizations control creation, updating, availability, suitability, protection, distribution, access, retrieval, retention, and disposition as needed for effectiveness.
How much is enough?
Extent depends on size, complexity, process competence needs, and risk. Over-documentation creates version chaos; under-documentation fails to demonstrate conformity.
